Dear Prospective Client,
We look forward to assisting you with your health goals. There are two forms you will need to complete prior to your visit and the links are below. As a licensed Doctor of Pastoral Medicine, regulations require that all prospective clients review and sign the Pastoral Medical Association's Member Services Agreement (MSA) form before we are able to provide services to you. This form protects your right to seek pastoral health care services, and our right to provide them to you. This form must be received in our office "before" your first appointment. For your convenience you can complete the form online at the link below. PLEASE NOTE THAT THIS FORM, MSA, MUST BE COMPLETED PRIOR TO SEEING DR. HOOVER!
Note that on the bottom of the form under Additional Information you will need to insert the practitioner name so that we will be notified after you complete the form. Please insert the following information:
